Little information about the life and work of industrial safety pioneer Herbert William Heinrich is available nowadays; most discussions and presentations of his work are based on derivatives and later interpretations of his work, or approach the matter from merely one perspective. Included are Heinrich’s well-known ideas and metaphors such as the accident triangle, the accident sequence (dominoes), the hidden cost of accidents, the role of the worker and management responsibility. The final chapter features a discussion of Heinrich’s relevance today.
